how do I disable the LDAP Directory

Each time I boot up Outlook, I get a window titled "Microsoft LDAP Directory"
with fields as follows:
Server Name : NULL
Port : 3268
User Name : NULL
Password : 'blank'
OK / Cancel

How do I disable this?

State Outlook version and what address book service you use. Do you use an
LDAP?
