Outlook - Updating Meeting Planners/Appointments

When updating meeting as a delegate to someone's calendar,
some of the recipients either do not receive the updates,
receive duplicate updates, or a cancellation gets sent out
with the update but no cancellation was initiated. The
calendar of the user (not the delegate) has a black berry
but no palm pilot in case that is information that needs
to be mentioned. The delegate gets to the user's calendar
by using "Open Other User's Folder".

Does anyone have any ideas of what could be causing this?
Neither the delegate nor I can duplicate the issue at will.

Additinal information to the below is we are using Outlook
2000 & Exchange 2000.
