outlook toolbar problem

Good afternoon,

I have a problem that my technician doesnt seem to be able
to solve.

My bosses' outlook toolbars reset back to the basic ones
everytime she closes and reopens outlook. We also run
customise toolbars and we lose them (and the shortcut to
relink them everytime.

She says a pop up question MIGHT have come up and she just
clicked (not sure yes or no) then watched her toolbars
disappear.

Does anyone have a solution to this, it happened 2 weeks
ago.

Menu/toolbar modifications are stored in a file named outcmd.dat. Couple of
things that I can think that might cause issues are...

1) Roaming profile
2) Mandatory profile

I think I first start by renaming outcmd.dat to outcmd.bak while Outlook is
closed just to rule out this file being corrupted or on a weak spot on the
hard drive before trying to figure out if 1 or 2 might be the cause.
