Outlook sending 2 Appointments when editing Existing One

I have a user who when Makes changes to an existing appointment and click
send update the destination users first get a mail which is cancelation of
the changed appointment and then a new appointment with the changed values in
them.

What could be the issue?

Please refer to the Answers below.
1. The Outlook and Exchange Server Version - Outlook 2007 Exchange 2003.
2. Whether the issue can always be reproduced - No it happens randomly.
 
Actually i have found out that when we a meeting request to a group that the
sender also is a member of and the recipient accepts it it get deleted from
the recipients calendar.

It seems to be a bug in 2007 ?

I have performed some further test regarding the issue.

Scenario 1: Exchange 2007 and Outlook 2007:
=======================
1. Have UserA created a meeting request and sent to GroupA (including
UserB) and UserB.
2. Have UserA update the meeting request by removing UserB and select "Send
updates to all attendees"
3. The UserB receives only one message indicated that the meeting is updated

Scenario 2: Exchange 2003 and Outlook 2007:
============================
1. Have UserA created a meeting request and sent to GroupA (including
UserB) and UserB.
2. Have UserA update the meeting request by removing UserB and select "Send
updates to all attendees"
3. The UserB receives two meeting message. One message cancel the meeting
and another message have the updated meeting request.

I would like to explain that the different behaviors occur because in
Exchange 2007 and Outlook 2007, duplicate invitees are removed from the
attendee list without your intervention. This functionality comes from new
attendee list scrubber logic. When an organizer clicks on Send or Save and
Close for a meeting item the following logic is executed:

1. Treat each DL and each invitee as an individual entry in the invitee
list, and remove duplicate invitees in the list.
2. Send an update to the existing invitees and do not send cancellations to
the removed duplicate invitees.
