Outlook Issues for a Perticular User

It seems to be weird that a perticular user in my company has some outlook
related issues. Issues are as follows.

1. User has complained that her Mails in her inbox has got deleted
automatically.
2. When this user sends out Calander invites and the Receipient accepts the
Invite it gets deleted in the Receipents Calander.
3. When the user changes an existing appointment and send update the
receipent receives a cancelation of appointment first and a new appointment
with the changed values.

What could be the Issue ?

Hard to say without any configuration information.
It could be that the user is configured with an Exchange account but
delivery is set to a pst-file instead of the Exchange mailbox. Just a wild
guess though.

No the user is having a OST file and Outlook is in cashed mode.
