Outlook Phone Message Form

Our Company uses Outlook 2003...updating is not an option.
I used an outlook form to advise recipient they had received a phone call.
It had a place for all the pertinent info and a text box to type in any
message left.
My computer went haywire recently and the form was corrupted and ultimately
deleted by IT. I cannot find a similar form anywhere to replace this lost
form that I used daily. Thanks for any help you can provide.

Thanks I tried that one, but when I went to personal forms, it was not on the
list even after I downloaded it twice. Maybe it doesn't work with 2003? I
don't know what it looks like so am not sure that's the one I need anyway.
The one I've been using was 'You received a phone call, please .....

It works with Outlook 2003 as well. You must connect the pst-file included
in the download to Outlook and access the form from there.

You might have success with Telephone Message Pad (http://www.TelephoneMessagePad.com).

There are step-by step instructions for integrating with Outlook (http://www.TelephoneMessagePad.com/outlook_tb.py). While the pictures are Outlook 2007, the same instructions work for Outlook 2003.
