Outlook loses signatures

When requesting a new email in Outlook, we frequently see the error
message:

"The signature "xxx" could not be located. This signature may have been
removed."

This happens sporadically, anywhere from once a month to daily.

Sometimes they can just go to Tools>Options>Mail Format and just
re-select their signature. Other times there are no signatures listed
there, and then they have to re-start Outlook, OR log out
and log in again, OR reboot the machine, OR (last resort) recreate
their signature file from scratch.

In all cases, the signature file still exists in Documents and
Settings\user\Application Data\Microsoft\Signatures, but for some
reason Outlook doesn't see it.

How do we get this to stop?
 
It is plain text that has been typed into the Tools>Options>Mail
Format>Signature Picker>Edit window. Nothing has been imported, there
are no images, just text.
 
No. If it comes back by itself after a re-login or re-boot, it's fine
for a few days - or a few weeks. If we remove and re-create it, it's
fine for a few days - or a few weeks . . .

A new wrinkle - one of my users sent an email this morning and Outlook
gave her the error message "signature may have been removed". She
ignored it (they're used to this by now). When she sent another email
an hour later, it was miraculously restored. She didn't re-set or
re-create it; she didn't even re-start Outlook.

We have our accounts set to send everything in plain text, so could
this be an issue?
