Outlook forms

I have created a customised form in OutLook 2003 pro. but when I send the
form an empty message is sent only
What is the problem?

Most common causes:

1) The form was not published to the Organizational Forms library.

2) THe form was published but has the "Send form definition with item" box checked.

3) The form designer did not click the Edit Read Page button and design a read layout.
