Outlook crashing when accessing public folder of any kind

I have Outlook 2003 sp3 installed on WinXP machine. I have a Outlook COM
Add-In added, which pulls custom forms from Organisational folder.

Whenever user tries to access public folder of any kind, outlook crashes.
Below is the event log for the same:
Faulting application outlook.exe, version 11.0.8169.0, stamp 465f28e3,
faulting module ntdll.dll, version 5.1.2600.2180, stamp 411096b4, debug? 0,
fault address 0x0000eddc.

Pls advise.
