Outlook crashes when accessing public folders

Outlook crashes when accessing public folder of any kind. I have Outlook 2003 sp3 on Windows Xp machine. I also have COM AddIn installed on my machine, which basically pulls custom forms from organisational folder.

The error from event log is as below:
Faulting application outlook.exe, version 11.0.8169.0, stamp 465f28e3, faulting module ntdll.dll, version 5.1.2600.2180, stamp 411096b4, debug? 0, fault address 0x0000eddc.

Any help is appreciated.

Have you tried creating a new mail profile via the mail applet in the
control panel? (don't copy the existing profile, create a new one and see if
the crash happens under the new profile.)

New profile didnt helped but diabling the Exchange Cache Mode did worked.

But this workaround is cumbersome, as it would have to be done across the board for all users. Is their any other workaround, which might have worked for others?

The version number you supplied suggest you have Office 2003 Service Pack 3
installed, does the issue stop once you roll back to Office 2003 Service
Pack 2? Reason I ask is for two reasons...

1) There is a known issue about COM Addins and Service Pack 3

2) There are changes in regards to custom forms

If you can't roll back, then you would have to isolate if the cause is by
the COM Addins by disabling all of them. (e.g. Can you access public
folders in cached mode by starting Outlook 2003 in safe mode?)

Other than that, I can access the public folder store on Exchange 2003 while
Outlook 2003 SP3 is in cached mode. Difference is, I don't have any 3rd
party addins installed.

As per Issue of COM Add-Ins with Office sp3, I am not getting event ID 21296 in Event Viewer as suggested in knowledge base. So it seems, crash is for some other reason.

However, if I disable my COM Add-In(which creates a toolbar, which pulls custom forms published in Organizational folder); I am able to access any of the public folders. While it crashes otherwise, if Add-in is enabled to load at startup.
But interesting point is, this is intermittent behavior across the board. Its seems to be working for some users but it crashes for some with all having sp3 on their systems.

This is driving crazy. Pls can you give any direction to this.

There isn't enough information to help. (By this I mean, I don't know how
things are implemented, what other things your site might have that
intergrate with Outlook, .etc. This is an internal solution where it
requires time and access to source code to isolate the cause.)
