Outlook check box actions

Is it possible to insert a check box into an email and when the recipient checks the box and clicks SUBMIT it will automatically send response to the sender?

Sending over the Internet? no, because Outlook doesn't run code on email
messages.

Sending within an Exchange organization? Maybe, with a custom form.
