Outlook Appointment Disappearing

Hi,

I used my custom Calendar form to create appointment, but as soon as saved
the appointment, it disappears. I find it in my inbox "Sync Issues" folder,
there I find all that disappeared appointments.

Could anyone tell me how to resolve this issue?

Here is the message in the inbox:

I have syncronized the calendar folder based on instruction, now the item
count on server folder and offline folder are same.

But Calendar Items are still moved to "Sync Issue" and "Local Failures"
after few senconds.

Any idea?
