Outlook 2003 forms- troubleshooting

Good morning

When I create a new form, it looks perfect, the fields are all in place,
etc, but when I actual e-mail it (to myself and a colleague as a test), none
of the fields that I have added (even if they are filled in) can been seen.
So, when I send my form, filled in, I only see the sender, receiver and
subject (like an e-mail). The form is the based on the standard 'message'
template.
This form is used to check in and return books in our resource library
(e.g.- Book #, Book title, fields are not seen when e-mailed)
 
The most common message form mistakes are:

-- not creating a read layout with controls bound to the same fields as the controls on the compose layout
-- not publishing the form where everyone can use it.
