Outlook 2003 - Create and Share Forms

How would I create a form and share it with other outlook users?

Save it as an oft-file or publish it in a Public Folder when you connect to
Exchange.

What kind of form (message, contact, etc.)? For what purpose? Does your
organization use Exchange for mail?

FYI, there is a newsgroup specifically for Outlook forms issues "down the
hall" at microsoft.public.outlook.program_forms or, via web interface, at
