Just swinging by to let you know that I finally have resolved this after
much frustration! This is what I think did it. Whenever I would go to
fax something, the Fax Console would open with the Send Fax Wizard. I
choose 'cancel' and told it NOT to use that next time I sent a fax.
Then, when I went to print something to the fax device again, I got my
normal Send Fax window WITH my Address Book button in there that
connects directly to Outlook. So, my advise to anyone else is to make
sure you have that startup option for the Fax Console's wizard turned
off.

Whew! Now what happen to my list of other challenges & annoyances...

Most likely another in the long list of registry settings that fail to get
changed when you upgrade over IMO. The Address Book in Word won't work
either in that scenario.
I always recommend clean installs when it comes to Office.
