Outlook 2003 adding unwanted attachments

I have a user who is running Outlook 2003 and sometimes when she sends out
email it will contain attachments that she didn't attach. I've done a full
virus scan on her machine and came up empty. Has anyone else ever heard of
this?
 
what is the attachment? if html or txt and she uses word as her editor, it
could be a header or footer enabled on the word template.

otherwise, is she using a signature with images in it?
