open form when pdf document opens

Hi,

I want to associate pdf documents with my application, when a pdf
document is open i want my application to respond in by opening a form
for the specific pdf doc (i.e. to make notes with).

how can i do this? (maybe also for web pages!)

Usually it is done by adding special keys into Windows registry.
