Online Application

I am looking to create an online application in FrontPage. I would need for
the application to be read only so once the applicant puts his information
in, it will reset and the information will be sent to someone via email.
Any instructions on this?
 
Just create the form in FP, and set it to be send to an email address, this
requires that the host server has the FP extensions installed, that you
publish the web to the server via FP's http mode and that the host has
configured the FP extensions to process the form data via email.
