Old problem: Floppy drive reports unformatted diskette. please help.

I know there's an answer for this problem, becasue I saved it somewhere,
but now I can't find it. :-(

My grandson reports that his WinXP Home system on inserting a floppy
diskette has begun giving error message "Diskette is unformatted. Do you
want to format it now?" or words to that effect.

Would someone explain why, and provide a fix?

He has no patches or service pack on his system.
