Office-Word 2003-SP1 - How to create book header

I'm not sure what to call this except a header.

In regular print books, the book title is often at the top of every page.

In regular print books, often the chapter title is centered as well at the
top of that chapter's pages.

How do I do each of these / both of these in Word?

I'm not quite sure about your first post: Word certainly does not create
empty lines on its own. Sounds a lot more like empty paragraphs in your
document (something to be avoided at all costs, especially in longer
and/or structured documents).

What you are describing is called a "running head" in bookmaking
terminology. For this you use the document header in Word. See these
