NTBACKUP vs RSM Mount, Dismount, Eject, Re-Request Same Tape

I recently began experiencing the following during
attempts to restore data from tapes (used to work fine).

During NTBACKUP restore, select the proper tape and
NTBACKUP asks for the last tape in the backup set (for the
catalog) and issues the mount request to RSM.
RSM mounts the tape, NTBACKUP reads the catalog.
Then... for reasons unknown, RSM dismounts and ejects the
tape... and another inject is requested for the same tape.
The tape is injected, mounted, dismounted, ejected and re-
requested repeatedly - until the restore request is
cancelled.

Very weird... especially since restores used to work fine?

Thought it might be related to a messed up media pool, so
I created a new pool, moved the media into the new pool -
same deal.

Any ideas?
