Non-stop hard drive accessing..............??

Forgive the ignorance but I must ask, Can anyone lend a reason why my pc
(within one year old) is constantly accessing the hard drive? This problems
makes for opening any application close to a 2 min process. Thought?
Solutions appreciated......

system specs? running processes? Increase ram/disabling indexing service.
