Newbie Question on Forms and results

I am trying to add a customer survey form to our existing
website using Frontpage 2000. I had no trouble creating
the form in Frontpage, and no trouble copying it to our
web server via ftp. The page opens fine, but when I
click "submit", I get the "The page cannot be found"
page. Also, the data from the form fields is not being
added to the file I specified. My web host says they have
the Frontpage extensions. Is there something I need to do
to activate the forms capabilities?

You must publish the form to the server via FP's http mode. Your host may need to re-install the FP
extensions on your account since you have use FTP to upload content, as in many cases using FTP will
corrupt the extensions.
