New form does not display form body

I created a new form based upon the standard message form. I entered all of
the components that I wanted and published it to my personal folder. When I
select the form to send a new message, only the address line appears and not
the form body. What have I done wrong?
 
By the "address line," do you mean to To and Cc blocks? Did you remove the item body control during your design session?
