Need to save forms as an editable document

I have prepared forms for keeping records, and the forms have a lot of drop
down menu items for speed and convenience. There are also some text fields,
and non-editable text (when the document is protected).

Is there a way to fill out the fields as a protected document (since filling
out the fields cannot be done unless the document is protected) and then
print out or save as a document that can be edited / polished?

For instance, I may have the following:

Child is 2 years old with the diagnosis of: ear infection.

In this example, the number "2" would be chosen from a drop down menu with
numbers from 0 to 18, and the "ear infection" would be an exitable text box.
The other words would be noneditable text.

Is there any way to save this document in a format that would eventually
allow me to edit the entire sentence as needed (e.g., as an RTF docuemnt that
no longer has the fields)?

What happens if you select the document and press Ctrl+Shift+F9?

Absolutely nothing happens. I opened the document in MS Word and hit that key
combination with no response or activity. I clicked on/selected the file (in
its parent folder) and hit the key combination with no response or activity.
What should have happened?

Try Ctrl+A (Select All, which is what I meant by "select the document"),
then Ctrl+Shift+9. Ctrl+Shift+F9 unlinks fields (including form fields, I
would expect).
