multiple mails arriving from senders

I have an Exchange 2003 environment, and an internal user sends a mail to
another user via outlook 2003. On the server logs it shows that it has
delivered it to the mailbox, and from the senders mailbox it shows that it
has sent it. both of the reports also show that it has only been sent once.
but yet on the recpient side, it show four times.
 
Try on the following...

Make sure you dont have duplicate rules in outlook.
Make sure in the process only one outlook.exe is running.
 
Only one instant of outlook is running, so there is only one process running.
How would I check to see if there is a duplicate rule?
