Multiple items on a form

I am working on a form that tracks part numbers and costs. The form
originally had only one line to input information from an invoice. I am
trying to come up with the easiest way to create multiple input lines of the
same category on my form without creating any new fields within the table.
Can someone help me?

date part no description price qty total

would like to create new line item in the table for each line item in the
form using the same field names in the table.

Your best bet would be to use a continuous form.
