MSWord TOC entries as hyperlinks when printing to PDF

Using MS Office Word 2003: I want to convert the Word doc so that its TOC
page entries or heading entries (or both) are functioning as hyperlinks in
the PDF file. How to accomplish this?
 
The answer depends on what PDF program you're using. Try
searching the support website for your PDF program.
