MS Outlook 2003 "insert" vs "attach" question

A friend of mine sends me photos actually inserted in her email document.
When I try to do the same, they are always sent as 'attachments' and not
inserted in the document itself.
I changed one of the options to send via html email but that didn't help.
Can anyone here tell me how to 'insert' instead of 'attach'?
