Modem Driver Help please

I have one JW JetWay modem, external, and the only other markings are on the
bottom bar code: 1AM130008510, and a silver metallic sticker under the last:
CE 0700 X.

I seem to remember using this as a data modem, and voice, and Faxmodem as
well.

Posted for help a while ago, as my internal modem is giving trouble, so now
found this one again, any one any idea on a driver which may work??

My operating system is Win2000 Pro.

Another problem is that I have a number of power supplies to hand, and no
idea which is the correct one for its. some have centre pin positive, others
outer ring positive. Again any help here would be great?

I'm in the UK, and only require the modem for Fax, will use it a far bit but
hate to see the old thing just dumped as no driver.
