Microsoft LDAP Directory POPUP

Everytime I open Outlook this thing popups called "Microsoft LDAP Directory".
How do I get rid of it? It asks me for a "server name", a "port". How do I
get rid of it?

Open Outlook and select Tools > Email Accounts > View/Change existing
directories and address books and then remove the LDAP address book.
