Yes, they are syncing with a Blackberry. Yes, it seems to only happen when
they send updates. A common denominator is that these are all delegates for

I too am having the same issue and it appears to be only updates. This
involves an organizer and a manager who uses a Blackberry with a blackberry
server. They were both originally on Outlook 2003 and I upgraded the
organizer to 2010 to see if that helped any and the same issue happened
again. Exchange is 2003 6.5 Build 7638.2 SP2. BB server is Version
4.1.5.26. Has there been any solution to this yet?

Best guess based on Outlook Calendar mechanics... If it's not the phone syncing, then it could be that the intended attendee is not fully accepting (or marking tentative) the meeting request. Thus, when updates are sent, that person is not on the attendee list so the update is not sent to them and the original meeting calendar item is deleted on update. Let me explain further.
When a calendar item/meeting is created and sent to intended attendees, Outlook stores a record of a meeting and the list of attendees that accepted it LOCALLY on the PC of the person that created the meeting NOT on the exchange server. Thus, when a meeting is updated, Outlook checks the locally stored list of accepted attendees and sends an UPDATED calendar item/meeting to all the accepted attendees then removes/deletes the original item/meeting. The updates are ONLY sent to those that are marked as accepted or tentative so those that did not accept don't see the updates. And, because the original meeting is deleted once update, those that did not accept suddenly see that the meeting has disappeared....
