Macro document is blank

I have a word doc containing macros that look great and prints great on my
PC. When it is e-mailed to a collegue, it arrives blank. That is all the
boxes I filled in before I sent it are empty when it is opened on the other
PC. Any clues as to what is going on?
 
What you are seeing could be caused by macro security, which disables most
macros in Word. However, I think we need some more information...

Is this a form document?

Which version of Word?
