Locked out for no reason

I didn't even set up my security protocols, and some how I have a dialog box
prompting me for my password. It has a name and password box, but I never
set up a login with my name on it, but there it is? How does this happen?
I'm locked out of Access all together. What the heck am I supposed to do?

Bugs.
Did you try pressing enter with a blank password.
This has happened to me. Only in Windows not Access.

Open Access, without opening any database. Select Tools... Security...
Workgroup Manager from the menu. You should get a dialog box with the name of
a MDW (workgroup security) file.

If this is SYSTEM.MDW, probably in C:\Windows\System or \System32, then
someone has incorrectly implemented security on this or some other database.
You'll need to find out who and how and get at least some password (well,
first try userid ADMIN with a blank password, that's the default).

If it is some different file, choose the option to join another MDW file and
find the SYSTEM.MDW file and join it.
