Did you install the device in the following way:
1. Connect the device to the gameport
2. Start -> Run -> Control Panel
3. Switch to "Classic View".
4. Run the Gaming Options control
5. In Gaming Options Click the "Add..." button.
6. From the list choose "Microsoft SideWinder (Auto Detect)"
7. This should install your device.

If do what kind of sound card do you have? Do you have more than one
gameport reported in Device Manager? Can you post a DXDiag report of your
system so I can see if it has an indication of what might be wrong?

DXDIAG:
1. Click Start -> Run
2. Type "dxdiag" without the quotes and press Ok.
3. In the DirectX Diagnostic tool click "Save All Information..."
