jewel case booklet

I understand the basics for creating booklet in Word 2003. I've been doing it
for our Church every Christmas. What I'm not sure of is how to set the page
layout so I can print a booklet roughly 4.75" x 9.5" for insertion into a CD
jewel case. It will have about ten pages. I found a couple MS templates but
they're only good for 2 pages. Adding pages seems to muck things up.
Suggestions.

Unless you're actually printing on the required paper size, you will need to
create the booklet at full size and reduce the text area with margins. You
will then have to trim the resulting printed pages. If you'll be printing
this yourself (from your desktop printer or using a copier), set it up with
the top of the booklet at the top of the page (to reduce one trim cut). If
you'll be sending it to a commercial printer, ask what he prefers. He may
want each page centered on a single sheet (not set up as a booklet at all,
since he'll be performing the imposition), or he may want a PDF set to the
finished size.
