Jaggy sound: where to pinpoint the problem and fix it?

(Excuse me if I'm the umpteenth to ask a recurring question.)

I experience "jaggy" playback of sound on many occasions.
By "jaggy" I mean: like both switching off and on (eight times per second, I
guess) and having an echo at the same time.

It seems to occur depending on the file format, but with another factor as
well.
It occurs when playing MIDI through a browser, but not when played through
(a lister plug-in of) Total Commander.
It occurs when playing a WMA-file through Total Commander or IrfanView (also
using a plug-in), but not when playing it through Nero Media Player. It does
not now using Windows Media Player, but I'd swear it did so the day before
yesterday.
I have not yet discovered any wrong when using "real" music software (like
sequencers).

My impression is, that occasionally the wrong codec gets picked, or the
right one with wrong parameters.
I checked and in turn disabled all audio codecs I found listed in Control
Panel | Sounds and Audio Devices | Properties | Hardware | Audio Codecs
Properties, but to no avail.

It hasn't always been so. I have the impression that this started right
after the installation of QuickTime Pro, when it was in v6.2 thereabout.

I don't expect anybody to find the exact answer for me. My question is:
where would I have to look in my system (XP Home on a 1.6GHz Centrino
notebook), or what keywords would I have to use in an Internet query?

I have the same problem, XP Pro, 1.6 Ghz Thinkpad T41, sometimes
happens, sometimes not (usually when plugged in to the wall).
Wierd stuff.
