After creating selection boxes (dropdown,check,etc) and then run form, the
selection returns to last item listed in the selection box regardless of the
selection made.

Are the controls (combobox and checkboxes) bound to saved fields in the
table? In other words, are you storing this value with each record?

Is your question actually about InfoPath? That's a different product.
