is it possible to generate an index that hyperlinks to
index entries in the document?

No. You can manually add hyperlinks from individual entries in the
index to bookmarks that you add in the document. But as soon as you
update the index field, all your hyperlinks will disappear.

This is a fairly frequently requested feature, but it hasn't been
included yet.
