Index for mulitiple documents

I am using a heading and subheading in various chapters, typed exactly the
same; however, the index heading is repeated for each chapter in the index
rather than just one with the sections and page numbers under. For example,
it looks like:

Installation
overview, xi
Installation
instructions, 1-1
Installation
FAQs, A-1

I want it to look like this:

Installation
overview, xi
instructions, 1-1
FAQs, A-1

Can you help me? I'm going nuts trying to figure out why this is happening.
HELP.

lil bit of cheating makes for a simple soution:


Create a new style called appendix heading
give it an outline numbered list of A etc
set its paragraph outline level to be level 2 (format > style > para -
see top right of dialog)


Appendix heading 2 uses outline level 3.


Stick a section break before the appendices, use a new heading 1 with
the text Appendices. Each appendix becomes a heading 2 and you get
some sense in your TOC. It is not quite what you asked for, but it is
a quick and easy compromise.
