Import to Contacts from Excel

Hello Everyone,

I created a separate custom form for another folder in my Contacts. I tried
to import some data from an Excel file, and it imports ok, but it defaults
to the Old Contact form. Is there anyway to import the data to my
user-defined custom form that I made??? Any help would be greatly
appreciated.
