Import from csv to contacts, some are not added

When importing from a csv, the import tool fails to add
some of the lines but I can't find an error. I reduced the
file to two lines, the list of fields and a line to be added.

CompanyName, Fname, Lname, email, PhWk, PhHm, PhCel, Title,
StreetAdd, StreetAdd1, City, State, Zip, Notes, LeadSource,
WebPage
"Entzzzy","Jeremy1","Trzzz1","(e-mail address removed)","222-
222-4800",,,"Cofounder.",,,"New
York","NY",,,"VP_Invest","www.entzzzy.com"

I am mapping notes, leadsource and webpage to user2, user3,
user4 because I can't seem to add them to the Notes and
WebPage fields.

Any ideas why this wouldn't work?

Some more info...

The contacts folder that I am importing to has 16383 items in it.
When I add a new record manually, I get the following error:
"The messaging interface has returned an unknown error..."

Any ideas?
