If I broke the Switchboard form can it be fixed?

I imported the switchboard form from another db but it can't seem to see the
items in the table Switchboard Items.

Can I get these two to talk or do I have to start over?

You need to import both the form and the table.

The tables need to be either linked or imported as well. I think there
are two of them, but I'm not sure.
