IE6 encountered an ADD-ON problem.....

Using IE6 updated in xp home sp2, I encountered this message 'IE has
encountered a problem on the ADD-ON' and need to close.
The following add-on was running when the problem occur

File: Flashad.ocx
Co. name: " Adobe System "
"Adode Flash Player"

I have uninstalled and re-installed the Adode Flash Player again and I get
the same message, In the event log, the error code is 1004 or 1000

When the message appears, I check its app name:iexplorer.exe mod name:
mshtml.dll. In all instances, the app name remains the same while the module
name may change sto such as ole32.dll

What can I do to get rid of the message and the system just got reformatted,

Uninstall flash. Go to macromedia flash website and download the latest
version. If the issue wtill persists send me the crash usrdump from the box
for analysis.

persist.
