IE Crashing when opened, perhaps related to ISA?

When I start IE I get an error box with an option to
report the problem to MS.

When I toggle the "Use proxy server box" start IE, then
Check it again with my servers info It works until next
boot. We are using MS ISA as the proxy server and this is
the only device experiencing this problem.

The OS is Windows XP with Office XP.
All updates and patches have been installed via
windowsupdate.

The event log contains these following:
Faulting application iexplore.exe, version 6.0.2800.1106,
faulting module winshow.dll, version 1.0.0.8, fault
address 0x00002156.
and
Faulting application iexplore.exe, version 6.0.2800.1106,
faulting module ntdll.dll, version 5.1.2600.1106, fault
address 0x0000234e.

Hi there,

Follow these steps:

1. Click Start, Run, type INETCPL.CPL and click OK.
2. Click the Advanced tab.
3. Under Browsing, clear the Enable third-party browser extensions (requires
restart) check box.
4. Restart Internet Explorer.

[NTDLL.DLL] could be a coolwebsearch variant, you may download coolwebsearch
trojan remover from here:
http://www.kellys-korner-xp.com/regs_edits/CWShredder.zip

Finally, download Ad-Aware from www.lavasoftusa.com, scan the system and
eliminate the malware products. Re-apply the above fix if necessary. Don't
forget to update the pattern file using WebUpdate in Ad-Aware. Here are the
steps:

1. Start Ad-Aware.
2. Click "Check for Updates", "Connect" and download the latest pattern
file if available. and click Finish
3. Click "Start" "Next" [System will now detect the spyware]
4. After the scan is finished, click "Next"
5. Select all the items (checkboxes)
Right click on any object in the "Scan result" list and choose
[Select all objects]
6. Click Next and click OK to confirm spyware removal.
