Icon Problem

My some of my icons suddenly have lost their normal picture and show that
blank window icon. I've tried restarting my computer and virus scan came up
with nothing. Any help on how this can be fixed?
 
What icons?
Are these shortcuts or program icons?
Where are the icons located?
What happens when you double click one of these icons, does it still work as expected?
Did this happen immediately after a new software program was installed?

Shortcuts to Programs such as:
Internet Explorer
McAfee (security, spam killer,Privacy)
Adobe Photoshop

Yet my Microsoft Word shortcut looks normal as does Office outlook, Excel.

All of them work if double clicked. The above mentioned are on my Desktop,
however if I click Start and look at the icons on the left (most recently
used) They Too are not normal.

To my knowledge ( i am the only one using the machine to any degree, and the
others don't really know how to load anything new) nothing new has been added.

No virus's found, DATS up to date, Spy Sweeper run regularly, nothing found.

How's that?

TweakUI has an option to repair icons. Also:

Increase Icon Cache (Line 121)
