ICON in windows explorer and quick launch

I have a windows 2000 running on a machine for years. Somehow the file icons
in windows explorer and quick launch were gone. Though some icons still show
in windows explorer for some file, the fold icon and everything in quick
lauch all show a black square. What caused this problem? How can it be
fixed? Thanks for any help.
