how to show last modified date, time, and person

I need to keep all my documents current and show last modified date, time,
and person who last modified it somewhere on the document. there has to be an
easier way then entering the field and updating it everytime i need it to be
automatic
 
Insert DOCPROPERTY fields where you want this information to show.
