how to prevent sent items to be duplicated in the sent items fold

All of my sent messages in Outlook show up in the sent items folder in
duplicate. How do I prevent that from happening?

To make it less of a uessing game, how about telling us:

version of Outlook
type of mail store used

Has it always behaved this way? If not, what did you do just before it
changed?
