how to add forms to website?

I am using Microsoft front page 2003, I am wanting to add a form to my site
that where I type the question and in a white box they fill it in, also once
these click submit how can I set it up to were it will go to my email.

With the web open in FrontPage,
File->New
In the task pane that opened in the right click "more page templates"
Double click FeedBack form

This will give you a basic form to edit - add and/or remove fields as
required.

Right Click anywhere in the form and choose Form Properties

If your server runs FrontPage Extensions and supports email from the
extensions (ask your host - some don't support this) type in the SINGLE
email address the form data is to go to.
Click OK
You may get a warning message about no email server - ignore this.

If email or extensions are not supported, you will have to write or find
your own form handler, (using whatever technologies your host supports) and
select Send to Other, leave the dropdown on Custom ISAPI, NSAPI ....
Click Options
In the Action box, type in the location for the form handler script.
Click OK
